This form is required to be submitted for the following situations:
Late Course Add
Late Withdrawal
Late Registration
Overload:
Students whose cumulative GPA is under 3.0 should submit this form and get approval from the reviewers.
Underload:
Students who want to take under 12 credits, which is full-time status, should submit this form and get approval from the reviewers.
Course Equivalency
Repeat Course 3rd or more times
Academic Reinstatement
Scholarship Reinstatement
Stony Book Visit: Visit Waiver, 1 Semester Visit, Visit Deferment
